A unilateral protrusion of the nictitating membrane and enophthalmos in a 19-month old Belgian Warmblood horse.

    Abstract

    Case Summary:

    A 19-month old Belgian Warmblood stallion was presented to the Department of Equine Sciences, Utrecht University, with a chronic and continuous protrusion of the nictitating membrane of the right eye since 4 months. The complaint had had an acute onset 4 months ago and the eye was topically treated with corticosteroids by the referring veterinarian to no effect.

    Upon ophthalmic examination the right eye showed protrusion of the nictitating membrane and an enophthalmos/retraction of the globe that worsened by excitation of the horse. There were no other ocular abnormalities. The horse had a mild head tilt to the right. Neurologic examination did not show any other abnormalities.

    On ultrasound examination the globes were equal in size and no abnormalities were seen other than a diffuse and homogeneous swelling of the right nictitating membrane, suggestive of slight edema and/or inflammation.

    Endoscopy of the guttural pouches was performed to further investigate the mild head tilt. In the right guttural pouch a large swelling was visible in the medial compartment at the level of the middle ear. The stylohyoid bone had a thickened appearance and the internal carotid artery seemed congested.

    CT-scan of the equine head was initially performed in the standing, sedated horse and 3 days later repeated under general anesthesia while intra-arterial contrast was applied. A mass-like expansile lesion in the region of the skull base was seen, with expansion intracranially. The internal carotid artery was dilated.

    Initial differential diagnosis included neoplasia or (congenital) vascular anomaly: intra-arterial contrast administration confirmed the latter. Due to the poor prognosis the horse was euthanized and histopathologic examination was performed and the diagnosis of aneurysmatic dilation of the internal carotid artery was confirmed.
